Consent form for multiple Aquatic Activities

Launceston Aquatic Centre – Wednesday, 1^st November 2023 (Grade 7/8) Thursday 2^nd November 2023 (Grade 9/10)

The School Swimming Carnival will be held at the Launceston Aquatic Centre, commencing at 10:00am and concluding at 2:30pm.

Attendance at the Swimming Carnival Day is compulsory, as is attendance on any other school day. Participation rates are always high as the carnival is designed to give every student multiple chances to get in the water regardless of their swimming ability eg. Novelty events at the shallow end and unlimited water slide use to run alongside traditional races.

Students will be bussed to the pool from QHS at 9.30am and return to school by 2.45pm.

The canteen will operate providing food and refreshments but bringing food and drink is also recommended.

Parents/Guardians are asked not to attend.

Normal school rules regarding conduct will apply. In the event of inappropriate behaviour by students, we will contact parents and ask for their child to be collected from the pool.

Please complete the below consent form for water based activities and submit as soon as possible. Failure to return the form will result in your child not attending the excursions listed.

I understand that water-based activities/swimming will be occurring and that staff with appropriate first aid, CPR and rescue qualifications will be present. In the event of accident or illness when it is impracticable or impossible to communicate with me, I understand that the teachers at the venue will arrange for my child to receive such medical or surgical treatment as may be deemed necessary, and agree to reimburse the school for any costs involved. *
